Zion Participation & Release Agreement

Each participant and/or parents/guardians must agree to the Zion Participation & Release Agreement Prior to participating in Zion Foundation programs, activities, or experiences. This agreement covers important information regarding participation, safety, medical needs, travel responsibilities, media release, code of conduct, experience selection, and scholarship opportunities. If the participant is under 18, a parent or legal guardian must review and complete the required consent sections. Please read each section carefully and provide accurate information before submitting the form. By completing the Zion Participation & Release Agreement, you agree that we will be able to contact you or your parents/guardian, regarding all future Zion Foundation experiences.

Participant Medical Information

  • Known Allergies / Medical Conditions

    • Participants must list any known allergies, medical conditions, dietary restrictions, or other health information Zion Foundation should be aware of.  If none, type "N/A"

  • Current Medications

    • Please list any medications the participant currently takes.  If none, type "N/A"

ASSUMPTION OF RISK & INFORMED CONSENT

  • Assumption of Risk:‍ ‍I understand that participation in a Zion Foundation experiences may involve travel, cultural activities, service projects, community activities, transportation, outdoor activities, religious gatherings, and other experiences that may involve risks including illness, injury, accidents, foodborne illness, theft, natural disasters, water-related hazards, and death.

RELEASE OF LIABILITY

  • I understand and agree to release Zion Foundation, its directors, officers, employees, volunteers, agents, partners, sponsors, and affiliates from claims or liability related to injury, illness, loss, damage, medical treatment, emergency care, evacuation, or death arising from participation in the experience/trip, to the extent permitted by law.

MEDICAL TREATMENT AUTHORIZATION

  • In the event of an emergency, I authorize Zion Foundation representatives to seek and consent to necessary medical treatment, hospitalization, medication, emergency care, or evacuation for myself or my child if under 18. I understand that I am responsible for medical, transportation, and evacuation expenses.

TRAVEL & INSURANCE RESPONSIBILITIES

  • I understand that I am responsible for obtaining required travel documents, including a valid passport and any required visas, immunizations, or other travel requirements. I understand that I am responsible for costs related to trip cancellation, delays, early return, medical care, or evacuation. Zion Foundation strongly recommends appropriate travel insurance. If required for the experience, participant is responsible for Valid passport, required visas or entry documents, required vaccinations or health documentation, parental travel authorization for minors, airline and immigration requirements, travel insurance, international medical coverage, trip cancellation and interruption coverage, emergency medical evacuation coverage.

SCHOLARSHIP & MEDALLION STATUS ACKNOWLEDGMENT

  • I understand that Zion Foundation's Medallion Status Program may provide opportunities for participants who reach Diamond Medallion Status to be considered for a Zion scholarship. I understand that Diamond Medallion Status does NOT guarantee a scholarship, a specific scholarship amount, or any other financial award. Scholarships may depend on available funding, eligibility requirements, program criteria, and other factors determined by Zion Foundation. I understand that participation in Zion programs and experiences does not guarantee Diamond Medallion Status or a Zion scholarship.

CODE OF CONDUCT

  • I agree to follow Zion Foundation policies, instructions, and codes of conduct throughout the trip. I understand that inappropriate, unlawful, or dangerous behavior may result in removal from the trip at my own expense and without refund.

GOVERNING LAW & SEVERABILITY

  • I understand that this agreement is intended to be governed by applicable laws and that if any portion of the agreement is found to be unenforceable, the remaining provisions will remain in effect to the extent permitted by law.

MEDIA RELEASE & CONSENT

  • Zion Foundation may photograph, film, record, or otherwise capture my image, voice, or participation during Zion programs and experiences. I give Zion Foundation permission to use these photos, videos, recordings, my voice, and related information for educational, promotional, awareness, fundraising, and other Zion Foundation purposes, including use in print, digital media, social media, websites, television, radio, and other media, in the United States or internationally. I understand that this permission is voluntary, is provided without compensation, and may allow Zion Foundation to use the materials without requesting additional permission each time.

FINAL ACKNOWLEDGMENT

  • I confirm that I have read and understand the Zion Foundation Participation & Release Agreement. I understand that participation is voluntary and that I am responsible for providing accurate information. If the participant is under 18, I confirm that I am the participant's parent or legal guardian and have authority to provide this consent.
